T&F Visitor Registration

As a participant in Fetch Collective Limited’s events, I hereby waive and release any and all rights and claims for damages or injuries arising out of, or in any way related to, my participation in any event administered by the Fetch Collective Limited, which I may have against the following parties: Fetch Collective Limited, K9 Frisbee Toss & Fetch League, Disc Dog Clubs, Club Captains, administrators and owners of any facilities hosting Fetch Collective Limited’s events. I certify that my canine(s) and I are healthy and fit to participate in The Fetch Collective Limited’s events. I hereby grant to the Fetch Collective Limited the right to record, broadcast, photograph, film and videotape our (I and my canine’s) performances and related interviews at the Event and to utilize such recordings, broadcasts, photographs, films or videotapes together with our names, images, likenesses, voices, and biographical information, for such publicity, promotional or commercial purposes as they may desire. I understand that Fetch Collective Limited may assign the right to use the recordings, broadcasts, images, photographs, film and videotape of my performance(s) together with our names, images, likenesses, voices and biographical information to such person or other entity as they may desire without my further consent or knowledge and I expressly agree to such assignment.
